Intake Process
Referral
Get a referral from your primary care physician, using our standard referral form.
Scheduling
The Moxie team will reach out to you about scheduling the initial evaluation. Your appointment time will be the time used for the plan of care following the initial evaluation.
Please choose a time that will work on a recurring basis if you or your child qualify for services.
Intake Documents
Once scheduled, you will be sent access to the patient portal to fill out intake forms.
Please complete all intake forms at least one week prior to the first scheduled appointment.
Initial Evaluation
The initial evaluation can take between 1-3 sessions to complete depending on individual clients and client tolerance for standardized assessments.
Receive Evaluation
Once assessment is complete, the clinician will take the time the following session to complete the evaluation write-up. You will receive a copy of the report via the patient portal.
Execute Plan of Care
Review evaluation report and discuss with clinicians the plan of care. This can include: regular services with our clinicians, family coaching, and/or THIRD THING.
